A Disability Benefits Questionnaire (DBQ) is a form created by the VA to document the severity of a specific medical condition. It supports your disability claim by aligning your symptoms with the VA's rating criteria, and a strong DBQ can even help you avoid a separate C&P exam.

We can only complete DBQs for conditions that do not require an in-person physical exam for accurate assessment. Some conditions — like orthopedic issues or most neurological conditions (with the exception of headaches) — typically require a physical evaluation to meet VA rating standards. For conditions that can be assessed through medical history and symptom reporting, we're able to complete the DBQ remotely.

A P&T (Permanent and Total) letter is a document available to veterans who are already 100% service connected.
